CMS-1500 Form Filler (1500FF)
Description:
1500FF is an application that allows a professional to print out filled CMS-1500 forms in the proper CMS-1500 form paper for insurance reimbursement.Initially, one must enter the relevant information for all the patients. This is saved in a simple database. To printout the forms, one has to type the name of the patient and the dates of visits and press a button. This creates a page that can be printed to the CMS-1500 form paper. Press another button and the information will be printed with information entered in the appropriate form fields.
The patient's data can be easily edited via the application and it can be viewed on a web browser (Safari, for instance).
NOTE I: If you are interested in using this app to work with other forms, please contact me (email below). This app can be customized to work with any kind of forms and as an added benefit you get to have an simple database with the information you want to input in the form.
Please note that if you are using Mac OS Snow Leopard or later and you want to fill a few forms, you can do that directly with Preview, no need of special software.
